Assessment: Skills and Practices: D: In my position as an Elementary School Counselor, I have made several referrals to community agencies, including Mental Health Services, assistance in accessing health care, and referrals to our district's Youth Services Team (YST).
Assessment: Knowledge: E: In my position as an Elementary School Counselor, I have built and facilitated a Student Support Team (SST), where we discussed specific students, assessing potential barriers to academic, career, and personal and social development. We then problem solved, based on our identification of these barriers.
